Elohim Healthcare Services – Direct Support Professional (Full Job Description):

Elohim Healthcare Services is a healthcare agency authorized by the New Jersey Division for Developmental Disabilities (DDD) to serve adults in New Jersey with developmental disabilities.  Our goal is to help these adults become more active participants in their home and community lives, realizing their full potential value as valued citizens.  The services we offer include Community Based Supports, Direct Respite Supports, and Community Inclusion Supports.

We hire Direct Support Professionals (DSPs) to help us provide these services to our clients and their families.  DSPs work in a variety of places including individual and family homes, group homes, supported apartments, day habilitation programs, and the community. DSPs support people with a variety of activities that include, but are not limited to:

·       Learning new skills

·       Completing personal care activities

·       Accomplishing daily living activities (e.g. cooking, cleaning, shopping…)

·       Ensuring health and safety

·       Facilitating community connection and relationships

·       Upholding individual rights

·       Assisting people to be part of the community

In addition a DSP is expected to:

·       Follow all agency policies and DDD service standards

·       Complete accurate and timely documentation (activity logs, incident reports, timesheets)

·       Report incidents, abuse, neglect, or exploitation immediately per DDD requirements

·       Maintain confidentiality and HIPAA compliance at all times

·       Participate in required training, including Rights, Abuse & Neglect, and Incident Management

Minimum Qualifications:

·       Must be at least 18 years old

·       High school diploma or equivalent

·       Successfully complete background checks, fingerprinting, and DDD-mandated training

·       Ability to work independently and as part of a team
